Intake Assessment Care Planning and Evaluating of Care Services
What you can expect during this process
All About Caring will:
- ask you to give your consent in writing so we can access, discuss and review your
personal information that we need in order to deliver the service you want.
- discuss your needs and wants
- discuss your expectations
- treat you fairly at all times
- assist you to understand anything that AAC needs to communicate to you. We can provide
a translator when needed.
- advise you of what services AAC can and cannot supply to you – including our on-call
service
- advise you of your Rights and Responsibilities
- advise you of AAC Rights and Responsibilities
- set outcomes/ goals you expect to achieve while receiving services from us. This
will be written in a detailed care plan that is easily understood by you and everyone
else involved in your care
- review these outcomes and your care plan at regular times throughout the year
- Ask you to agree on and sign your Care Plan and Service Agreement
- provide you with specifically trained and screened Attendant Care Workers who are
covered for Workers Compensation, Public liability and Professional indemnity insurances.
All About Caring will not:
- participate in any illegal activities
- agree to perform tasks that are not detailed on your care plan without reviewing
your needs first.
- tolerate bullying or harassment of any of their staff.
AAC expects that their service users will:
- be given as much as information as possible to ensure they are making informed choices
- agree to sign their Consent form Care Plan and Service Agreement
- give feedback about the services they are receiving
- communicate with AAC regarding anything that will effect of the services being delivered
eg Hospital Admission.
AAC expects that their service users will not:
- ask Attendant Care Workers to undertake any tasks that are not agreed on and signed
on your service agreement
- ask any AAC staff member to participate in any unethical behaviour
- ask any AAC staff to participate in any illegal activity including any Unsafe Occupational
Health and Safety activities.
